New Patient Registration

Please bring your medical card to the surgery. All newly registered patients will be asked to complete a simple questionnaire and       attend a new patient check-up/assessment. This will allow us to meet you and assess your medical needs. The Practice has a catchment radius of approximately 10 miles.

It is the policy of the Doctors of Garvagh Health Centre, that all persons will have the right to join the practice list regardless of medical condition, disability, religion, race, gender, social class, age, sexual orientation or appearance.

Quality of Care for Patients

This policy also applies in the treatment of our patients: all patients are entitled to the same quality of care. Our aim is to deliver the quality of care that we would wish for our families and ourselves.

Change of Personal Details

It is important to keep us informed of any changes to your personal details such as, your name, address or telephone number to keep our records up to date. We also inform Business Services Organisation for their records and any correspondence they need to send to you.
